New Patient Health History Forms - Required (4 sheets total)

This let’s us know the history and current state of your health. What questions, concerns, goals, regarding wellness can we help you with? Let us know!

** Patient Tip** Save time on your first visit by sending your Patient Health History Forms to us TODAY.  Simply click on the Download & Print Form text below.  On the upper right hand side of the window is a button that says "Highlight Fields".  Click on this button and you will be able to type in your personal information directly onto the form.  VERY IMPORTANT, you must print the forms.  It will not save your text.  If you really want to speed up the process, you can directly FAX the forms to our office.  When your computer asks you to select a printer, choose FAX instead (your computer must be set up to fax documents).  Our fax number is 970-375-1584.  If you get stuck, give us a call! 970-375-1590.
